What is a Physical Examination Form?
The Physical Examination Form is a medical document required by all Chinese universities for CSC Scholarship applicants. It includes a complete body check-up, lab tests, and a doctor’s certification. Without this form, your scholarship application cannot be processed.
Where to Get the Form?
You can download the blank Physical Examination Form (Foreigner Physical Examination Form) in PDF format. Print it and take it to your DHQ District Hospital or a government hospital.
